# How to Create Custom Contact Forms in Axumind | Clientlex

Learn how to create Axumind contact forms, customize enquiry fields, configure form links, embed forms on your website, and add them to your client portal.

Not every prospective client is ready to purchase a service immediately. Some may want to ask a question, request a quote, discuss a custom project, or get in touch with your agency before making a decision.

Axumind lets you create standalone **Contact Forms** for collecting these enquiries outside the normal service checkout process.

You can customize the form fields, add conditional rules, configure its public URL, and share it with prospective clients or existing customers.

## Creating a Contact Form

- Navigate to **Forms → Contact Forms**.

- Create a new contact form.

- Enter a name for the form.

- Configure the introductory information displayed to visitors.

- Add and arrange the fields you want clients to complete.

- Configure any applicable field rules.

- Review the form and make it available to visitors.

You can create different contact forms for different purposes, such as general enquiries, quote requests, consultation requests, and customer support.

## Customizing Your Contact Form

The contact form editor provides several areas for configuring how your form looks and behaves.

### Form Information

Use the form information settings to configure the details displayed to visitors.

**Name**

Give your form an internal name so you can identify it in your dashboard.

**Form Information**

Add a short introduction explaining what the form is for and what visitors should expect after submitting it.

For example:

"Have a question? Send us a message and we will get back to you."

**Show in Portal Sidebar**

Enable this option to add the contact form to your client portal sidebar navigation.

This is useful when you want existing clients to have an accessible way to contact your agency.

### Customizing the Form URL

Each contact form has its own link.

You can edit the form link to use a more recognizable address for your agency.

For example:

https://youragency.axumind.com/contact

The form editor also supports changing the link without invalidating existing ID-based links. This means previously shared links that use the form ID can continue working after you customize its URL.

## Adding and Editing Form Fields

Open the **Fields** section to customize the information your contact form collects.

You can add new fields, edit existing ones, and arrange them in the order you want visitors to complete them.

A typical contact form may include the following fields:

**Name**

Collect the visitor's first and last name.

**Contact Email**

Capture the email address where your agency can respond.

**Phone**

Allow visitors to provide a contact telephone number.

**Enquiry**

Use a dropdown field to let visitors select the reason for contacting your agency.

For example, you might offer options such as General Enquiry, Request a Quote, or Support.

**Subject**

Give visitors a place to summarize the purpose of their message.

**Message**

Provide a larger text field where visitors can explain their enquiry in more detail.

You can also add other fields depending on the information your agency needs.

### Making Fields Required

Each field can be configured as required where appropriate.

Use the **Required** setting for information your agency needs to receive with every submission, such as the visitor's name, contact email, and message.

Avoid requiring unnecessary information that might discourage visitors from completing the form.

## Configuring Form Rules

The **Rules** section lets you configure conditional form behavior.

Use rules to show or hide fields depending on the answers a visitor provides.

For example, a form might display additional questions when someone selects Request a Quote, while keeping those questions hidden for general enquiries.

This helps you collect more relevant information without showing every visitor the same set of questions.

## Sharing Your Contact Form

Once your form is ready, use the **Share** option to access its sharing options.

You can make the form available in several ways:

**Public Link**

Share a direct link that anyone can open to submit an enquiry.

You can place this link on your website, in email signatures, or in social media profiles.

**Public Link with Portal Login**

Use the portal-login option when you want visitors submitting the form to log into your client portal.

**Embed Code**

Use the generated iframe code to embed the contact form directly into an existing website page.

This is useful for Contact or Get a Quote pages where you want visitors to submit enquiries without leaving your website.

**Support Widget**

Use the available widget code to display a contact or support option on your website. Visitors can open the form through the widget.

## Managing Contact Form Submissions

Contact forms provide a way for prospects and clients to reach your agency without going through the service checkout process.

Submissions are handled through the associated enquiry and support workflow, allowing your team to review incoming requests and respond.

This makes contact forms useful for:

- General questions about your agency.

- Custom service and project enquiries.

- Quote requests.

- Pre-sales conversations.

- Client support requests.

## Reviewing Your Contact Form

Before sharing your form, test it from the visitor's perspective.

Check that:

- The form name and introductory message are correct.

- Required fields are configured properly.

- Dropdown options are relevant.

- The form link works.

- Conditional rules behave as expected.

- The form displays correctly when embedded or opened through its public link.

Once everything is ready, share the form through your preferred channels.
